Step: 1
I will create a square logo in this tutorial but you can off course make it any shape you want.
Start by selecting the Rounded Rectangle Tool and the only setting you have to adjust is the radius of the corners, I used 5 pixels for my shape.

To give this shape some more life you can add a gradient to it. Start by creating a new layer above the layer with your shape and ctrl + click in the layer thumbnail of your shape layer to get a selection with the same shape.
Before we start with the gradient set the foreground color to a darker shade of the color you used for the shape.
Select the Gradient Tool, and open the Gradient picker and select the gradient called Foreground to Transparency.
In the image to the left i have marked which gradient to use if you can't find it.
I applied my gradient from the bottom left corner toward the upper right corner, but you can try different styles.

Now it's time to add a subtle 3D effect to the logo, we are going to achieve this with the Drop shadow and Stroke options in Layer blending options.Right click your shape layer and select Blending options from the menu.
Bellow is the settings that I used.

Well this is not much of a logo; we need to add something that relates to your business or site.
This step will vary very much depending on for what you are creating a logo. I will create a logo for a fictive site called hands on tutorials, so I will add a hand to the logo.
Do this by using the Custom Shape Tool and in the Custom Shape Picker find the hand shape.
To the left is the result, the only thing I have done is rotated it a bit so that it fits better with the logo.

Now the part of the hand not inside the logo is above the drop shadow, we have to delete this part to make it look good again.
Before we can do this we need to rasterize the layer, do this by right clicking the hand shape layer and select Rasterize layer from the menu.
Now you can do a selection and hit delete above the part that overflows the logo shape.

Step: 6
Time to add the site or company name to the logo. I will place the text on the right side of the logo which is very common.
I have split my text into to rows to make the logo more compact, and the only special thing I have done here is to make sure that both lines of text are of equal length, this is done with a setting in the paragraph palette, which you can see to the left.
